Our Wound Nurse will be involved in the assessment, treatment, and management of acute and chronic wounds, including pressure injuries, diabetic ulcers, surgical wounds, and traumatic injuries.
We create personalized wound care plans and collaborate with your doctor to ensure effective healing while minimizing infection risks and preventing complications.

Stomal Therapy Nursing
Our STN offers specialised care and support for individuals with a stoma to promote their wellbeing.
We support appliance selection, skin care, and troubleshooting, while collaborating with your doctor to help you manage your condition independently, enhancing quality of life and lowering hospital readmissions.

Adult – Continence Advise
Our main objective is to enhance quality of life through expert advice, personalized care plans, and the promotion of independence for individuals with bladder and bowel control challenges.
We collaborate with caregivers and teams to provide education, recommend interventions, and support continence management. We assist with Indwelling, Intermittent and Suprapubic Catheterisation

Children’s Continence Advise
Our Specialist Nurse supports children and young people with bladder and bowel issues through assessment, tailored care plans, and family education to enhance continence and quality of life.
We collaborate with children, parents, schools, and healthcare providers to offer supportive, evidence-based interventions such as bell and Matt, while reducing stigma and empowering families through education.

Reporting & Recommendation
We collaborate with Service Providers to create clinical reports that meet NDIS and Aged Care standards, detailing participants’ continence needs and recommendations.
The Specialist Nurse for NDIS and Aged Care clients assesses and documents bladder and bowel health needs for funding and care planning.

Education & Training
Education and training in Wounds, Continence, and Stoma Nursing is essential for improving clinical practice and patient outcomes by providing high-quality education to healthcare staff, patients, and carers.
Our Nurses will create training programs and workshops, offering mentorship to keep clinical staff and clients informed on best practices and healthcare standards, thereby enhancing workforce capability and patient care excellence.

CWCS (Commonwealth Continuity of Support Programme) – Assisting clients who are not eligible for NDIS but require ongoing support.
DVA (Department of Veterans' Affairs) – Offering services to eligible veterans and their families.

Case Studies
Background:
A 72-year-old male with a history of chronic venous insufficiency presented with a non-healing venous leg ulcer on his left lower leg, present for over 6 months. The wound measured 6 cm x 4 cm, with moderate exudate, slough, and signs of local infection. The patient also had poorly controlled diabetes and limited mobility.
Assessment and Planning:
As the Wound Care Nurse, I conducted a comprehensive assessment, including wound characteristics, vascular status (ABPI), nutritional status, and psychosocial factors. I identified contributing factors such as poor glycaemic control, inadequate compression therapy, and non-adherence to previous care plans.
Interventions:
- Initiated a structured wound care plan using antimicrobial dressings to manage infection and promote debridement.
- Collaborated with the GP to optimise diabetes management and prescribed appropriate antibiotics.
- Reintroduced graduated compression therapy after confirming adequate arterial flow.
- Educated the patient and caregiver on wound care, hygiene, and the importance of leg elevation and mobility.
- Coordinated with a dietitian to address nutritional deficiencies.
- Scheduled regular follow-ups and adjusted the care plan based on wound progression.
Outcome:
Over 10 weeks, the wound showed significant improvement—reduction in size, healthy granulation tissue, and decreased exudate. By week 14, the wound had fully epithelialized. The patient reported improved comfort, mobility, and confidence in self-care.

This case highlighted the importance of a holistic, multidisciplinary approach and patient education in managing chronic wounds. My proactive coordination, evidence-based interventions, and consistent follow-up were key to achieving a successful outcome.

What to Expect During Your First Consultation
For our initial consultation, we request 1.5 hours to facilitate a thorough understanding of your requirements and preferences. Understanding your health needs and creating a personalised care plan that enables us to effectively plan for our subsequent visits.
Consecutive visits
Following the collection of requisite information during the initial visit, subsequent appointments will be charged based on durations of 30 minutes, 45 minutes, and 1 hour.
